Losing heads help!!

Recently I have notice that I am loosing heads on my Hammer coral. Its about 40 or so heads on it. It has done great but I am not sure why 100%. I am also loosing heads on my trumpet coral.
I run XR15's and I was running AB+. Switched to Apex Radion control and used one of the settings which I later found out was way low. I've switched back to AB+. But I still have losses. O I also noticed STN on one of my SPS. I have had fluctuations with my parameters in the past but it never caused an issue. Would this light issue be the reason for the loss of heads. Also I dose Tropic Marin Trace Elements. I stopped at one point when I saw issues then started again when I thought things were ok. Could that have been my issue? Please help

Nitrate 0 (It has always been zero but just started dosing Nitrate)

Can you describe the loss a bit more, or maybe a pic would be great, and any new additions? The hammer heads you lost, the disappear almost overnight leaving behind a white skeleton? Any BJD noticed?
