How does everything else look in your tank?

lighting?

Anything that could be bothering it?

How old is your tank and how long have you had the torches

I'm not much help on torches. Hopefully @Rocks reef will chime in.

My experience - they bail when they're not happy. Have you seen any pets or any fish/shrimp picking at them? I wouldn't suspect fish or shrimp die to the head looking healthy but it's always a consideration.

Couple questions.
What is your salinity?
How is your flow?
Do you know your PAR?
As mentioned..:have you seen pests?
What clean up crew do you have? Shrimp? Crabs?

No pest spotted, Clean up crew just cleaner shrimp and hermit
Hermit most of the time is roaming on sand bed. And shrimp seem never pick on torch
Par in that area is 150-180. But i check it when setup the tank almost 2 year ago. No change on light flow and other might stress it.
What I notice is, this one done with spliting and when tissue in between split head separated it seem separate from skeleton. I notice the other one have small section tissue not attach on their skeleton, just move it to low flow and hope it can grow tissue and reattach back.
The bail out head seems no problem this morning like nothing happen.
I try to glue it to fragplug but i think its zero chance to survive.

I wish I had an answer.. it just seems to happen time to time.. coral boring spinoid worms are notorious for bailout if you see them.

Update more than 2weeks after bail out. The tissue seems not problem and still little bit puffy. Can it survive?
Also other head who share the skeleton also bailed out arround 3 days ago. Didnt know whats wrong until now. Other torch still happy and parameter still solid. Would be epic if this one can survive and regrow the skeleton

I had one bail several years ago. It never did regrow a skeleton. The flesh will potentially stay healthy if it stays eating and pooping. Hopefully it will slowly start rebuilding a skeleton and survive but this was not my experience.
