I can’t tell from the photo, is the tail fin eroded/missing? Severe tail damage is often caused by bacterial infection after some damage.
Is your Lubbocks wrasse a new addition?

Edit: Also, how long has he been in the acclimation box/ container?
I’ve seen similar symptoms in fairy wrasses over the years due to damage from collection or injury from darting into something. Do you have a mesh lid on your DT?Not long, I moved him to the DT from the QT, worried the Params were bad and the little booger jumped out!
O.K., if you could post a short video, it might help confirm it, but it is possible that your wrasse is seeing a neurological problem that is sometimes seen with these fish when newly imported. The fish basically spins in a circle or otherwise swims oddly. It may try to eat, but will have difficulty getting the food. Nobody knows what causes this. I think it is a virus or internal bacteria. Some people think it is from the wrasse injuring its spinal column by running into something in the tank - I don't think it is that because the affected fish don't have external damage on their shouts. It's not damaged that I can tell, just collapsed together
