Hammer head coral

So about 2 months ago I got a hammerhead coral - It was doing great...the other day it fell off the rock (knocked off my my turbo snail I believe). It didn’t look fantastic after that. Now it was come detached from its exoskeleton and is just at the bottom of my tank. I don’t know what to do. Please advise.

I checked my water parameters. Ammonia 0, nitrites 0 and nitrates 0

If it is nestled there under the rock there is a slight possibility that it can regenerate a skeleton to support itself. Although not many have luck with it there are a few cases where it’s happened
 
Usually polyp bail out is a water chemistry issue. I would recheck numbers and make sure something didn’t significantly change. N03 at 0 is never a good thing either as Euphyllia like more nutrient rich water.
 
Its hard to say without more info.

What are all of your parameters including po4?

What lights and schedule do you use?
 
My other corals in my tank are doing well...I have Zoas, Duncan Coral and BTA...all well.
My fish and coral banded shrimp all look good, swimming well, eating.

My Phosphate is 0ppm ? How do you raise that to the recommended .50 to 1 which I have read is needed?

My calcium is 340

I have an Fluval Evo tank - the lighting came with it...I do 4 hours of white, 8 hours of blue.

Calcium level seems low, do you do weekly/bi-weekly water changes?
 
do rarely come back,agree cal is low,phos low,what about alk,ph stable at?? salinity?
