Help With Hammer :Photo

I need help identifying if this is bad. Is this die off or growth? Do I need to do something? All help is appreciated. I’m worried because it’s in the middle of all the tentacles.

That definitely doesn’t look good man. I’ve got the same hammer (same color, looks the same) and I’d be flipping if it did that. Hopefully someone can answer what the heck’s happening here. Looks kind of like die off to me but it being in the center is weird.

I’ve seen an entire head die before on other hammers but this is freaking me out. I love this guy. I bought him at reefa-palooza last year.
 
Does this hammer have other heads?
How do your other corals look?
Parameters? Lights?
Have you tried dipping it?
How long has it been like this? any changes?

I’ve had it for over a year. It has two heads. They weird thing is that that thing is in the middle of a head. I’ve seen other hammers heve heads die and usually the polyps shrink and you can tell immediately something is going wrong, but in this case the rest of the head appears fine. Polyps are extending fine.

My other corals look fine except a birdsnest and Raja rampage, but they have been going down hill for awhile.

Alk: 8.12 - Trident
Calc: 416 - Trident
Mag: 1360 - Trident
PO4: 0.01 - Hanna
NO3: 0 - Red Sea

3 XR30 GEN 4 Pros with diffusers - AB+ Max 65%

Have not dipped it.

Its been like that for a few weeks with no noticeable changes.
 
It reminds me of a feeding response appearance in a plate coral, large frogspawn, red ball of death coralmorph? and a meat coral. But their mouths would close eventually after eating.
Is this a wall hammer or branching?
Does the hammer fill out like its happy or are the tentacles somewhat retracted?
If the tentacles look irritated, and I didn't see a pest on it, my fall back with LPS was a short lugols dip. ...or have you tried placing a tiny food item on the mouth??? My frogspawn (avatar) will quickly consume fish food that lands on it.
