Puffer Help! Trauma or disease?

Please help. Have a tiny stars and stripes puffer in a 180 gallon with an even smaller huma huma trigger and lemon peel angel. Plenty of rock with hiding places. I noticed these large white areas on his back this morning. Is this a disease / parasite or is someone taking chunks out of him? Should I move him to my quarantine tank or is it too late for him?

This is my first "jerk" saltwater fish tank. My other tank is a peaceful reef 55 gallon. I recently set up a 40 gallon quarantine and put the puffer in there for now, but he looks rough. The above mentioned 3 fish have been in the 180 gallon together for over a week without any issues. How in the world do I add other fish if this is a simple case of someone else eating the puffer? The huma huma and the angel follow each other around like best friends and everyone behaved during feedings.

Unfortunately, the puffer didn't make it. I think I narrowed things down to the Lemon Peel Angelfish. The trigger continues to appear rather chill, but the angelfish repeatedly and rapidly attacked anything I added to the tank. He won't be a good fit in my other tank so I'm probably going to give him to my LFS.

To me, the trigger is still an option since triggers as they get bigger will get more aggressive. I don’t think triggers should be housed in anything less than an 8’ long by 4’ wide tank unless they’re the Xanthychys species because their territory in the wild is almost 6’x6’ for the huge triggers but I find Titans and similar triggers have a 10’x10’ claim.
