No other fish has had issues and I’ve had this fish for five(ish) months. When fish show a general "disheveled" look, the common diagnosis is flukes (Gyrodactylus). Do you ever see this fish scratch/flash? Any other fish in the same tank showing issues?

I’d be shocked if it’s malnourished. I literally watch it eat twice a day like physically see it eating and I give it reef fenzy food.IN addition to Jay's comments - it looks thin, malnourished and sick in general. Since then it’s color has returned to normal and it’s much more active!Midas blennies have a huge issue with eating well, but getting skinny. Here is a post I made about that:
Skinny Blenny issue
Skinny blennies: Midas blennies are much more difficult to maintain than their common presence in the pet trade would have you believe. They are very prone to starvation. It is unknown if this is due to not feeding them often enough, not feeding enough, feeding the wrong foods, or if internal...www.reef2reef.com
